Grants are available for charitable organisations who are receiving, or wishing to receive, referrals through the NHS Waiting Well Programmes in County Durham and Darlington. The funding aims to address issues facing patients who are waiting for surgery.
Size of grant: up to £5,000 over 12 months.
Fund geography: County Durham and Darlington
Programme details:
Each Waiting Well Programme provides packages of support through participating organisations to individuals awaiting planned surgery, such as hip or knee replacement, who are identified as either:
· Living in the top 20% of most deprived areas in the region and who are impacted by other issues such as diabetes, cardiovascular disease, a high BMI or are struggling to reduce smoking.
OR
· Having a learning disability.
This pilot project focuses on improving an individual’s preparation for surgery. Not all patients waiting for planned surgery are eligible for support; only those who have been referred by the NHS Waiting Well Programme into a project will be able to receive support.
