Chester le Street Primary Care Network Community Investment Fund supports community groups and organisations based in Chester le Street, with funding provided by the seven GP Practices in Chester le Street that comprise both local Federation (Chester le Health limited) and Primary Care Network (Chester le Health PCN).
Who can apply?
There are no limits to the size of organisation that can apply, however priority will go to those organisations that can evidence that they can deliver significant impact locally. Priority to the fund will be given to organisations operating and delivering projects in Chester le Street, Sacriston, Great Lumley and Pelton.
What type of funding can you apply for?
Up to £5,000 is available to support the following:
Projects that target health needs within communities in Chester le Street
Activities to address inequalities in health and well-being including poverty, poor mental health, and isolation
Organisations introducing or integrating social prescribing within their services
Supporting positive mental health in young people
Enriching later life for older people
Healthy initiatives for hard-pressed families and socially isolated people
Organisations that are supporting older people and other vulnerable groups affected by the COVID-19 outbreak
Additional costs of working remotely and adapting services to improve health & well-being being delivered in the wider community including volunteer costs
Initiatives working to combat poverty (e.g. initiatives to tackle food poverty or ‘holiday’ hunger etc.)
Advisory services that complement those being put in place by statutory authorities such a directing people to local services or listening on the phone and giving emotional support

There are no deadlines – applications can be made at any time.
Size of grants
Grants are available of up to £10,000 although it is envisaged that the majority of grant awards will be around £5,000
