This fund provides grants of up to £500 to provide equipment and / or respite care for children and young people who have a disability or a life shortening illness, in line with the fund holder’s wishes.

Although this application form states the fund is for those with Cerebral Palsy, NFDC is open to children and young people with any severe disabilites or life shortening illnesses so please ignore this.
Under the question asking if you have Cerebral Palsy please state what disability / illness you have.
