The focus of the Healthy Communities Programme is to develop innovative community-led schemes in County Durham that will transform the actions of local communities to improve their health and wellbeing. The aim is to change cultures and behaviours around eating, nutrition and activity that will lead to improved health outcomes, reduced obesity levels and to a longer life.


The Healthy Communities Project aims to help communities improve their overall wellbeing (with particular emphasis on healthy weight and increased activity), and applicants are strongly encouraged to use the framework of Five Ways to Wellbeing to structure both their project and their application for funding.

Five Ways to Wellbeing is a set of evidence-based actions that promote people’s wellbeing and has been used by health organisations, schools and community projects across the UK and around the world to help people take action to improve their wellbeing - particularly feeling good and functioning well. Actions include simple things that individuals can do in their everyday lives.

Five Ways to Wellbeing are:

  • Be Active
  • Take Notice
  • Keep Learning
  • Give

Five Ways to Wellbeing aim to encourage people to be active, eat healthily and to be socially engaged. You will need to demonstrate how your project encompasses activities to challenge obesity levels, inactivity and poor diet, addressing particular challenges in County Durham. Projects will also need to demonstrate that they are targeting one of the core aims of the project, and will need to demonstrate their sustainability once the funding period has ended. Successful applicants will be required to submit monitoring forms to show how funding has been used to the benefit of communities, and take part in any promotional and/or publicity material.

Please note that you if you held a DCC Health Communities grant in 2017 you will be ineligible to apply to the Fund.
